Community-based care can help participants receive appropriate nursing support while remaining connected to their home and community.
Depending on the participant’s approved supports and individual requirements, support may include:

- Monitoring and observation
- Assistance with health routines
- Support with complex care needs where appropriately qualified staff are available
- Coordination with relevant healthcare professionals
- Support with maintaining appropriate care routines
- Assistance related to individual care plans
A Person-Centred Approach

We work collaboratively with participants and, where appropriate, their families, carers and other professionals involved in their support.
